I acknowledge by signing this agreement I have been given the full opportunity to ask any questions; which I might have about obtaining a tattoo from the artist performing my work.

I agree that all of my questions have been answered to my full and total satisfaction.
I specifically acknowledge that I have been advised of the facts and matters set forth below, and agree as follows.

I acknowledge by signing this agreement I have been given the full opportunity to ask any questions; which I might have about obtaining a tattoo from the artist performing my work. I agree that all of my questions have been answered to my full and total satisfaction. I specifically acknowledge that I have been advised of the facts and matters set forth below, and agree to the Art Bound Studio 18+ Tattoo Consent Form.

I am not a hemophiliac. I do not have diabetes, epilepsy, HIV, Aids, or any other communicable disease.

I am not under the influence of alcohol and/or drugs.

I acknowledge that it is not reasonably responsible for Art Bound Studio, or the artist performing the work, to determine whether an allergic reaction to the dyes, pigments, or process needed for my tattoo will happen. I agree to accept the risk if such a reaction should occur.

I acknowledge that infection is always possible as a result of obtaining a tattoo; in the event that I don’t take proper care of my tattoo. I agree to follow all instructions concerning the care of my own tattoo while its healing. I agree that any touch up work will be done at my expense.

I realize that any variations in color and design may exist between any tattoo selected by me, and as ultimately applied to my body. I understand that my skin color will affect the color of my pigment.

I acknowledge that a tattoo is a permanent change to my appearance; and that no representations have been made to me as to the ability to later change or remove my tattoo.

I don’t have any physical, mental, or medicinal impairment or disability; which might affect my wellbeing as a direct or non-direct result of my decision to have any tattoo related work done at this time.

I acknowledge that I have truthfully represented to Art Bound Studio, and the Artist performing the work, that I am over 18 years old, and that the following information is true and correct.

I acknowledge that obtaining my tattoo is my choice alone, and I consent to the application of the tattoo. I also consent to the action or conduct of the Artist performing the work, reasonably necessary to perform the tattoo procedure.

I agree for myself, my heirs, assigns, and the legal representation to release and forever discharge or hold Art Bound Studio, and the Artist performing the work from any and all claims, damages, and legal action arising from, or connected in anyway with my tattoo, and of the procedure or conduct used to apply my tattoo.

I am also aware my Artist is a 1099 Contractor and is Self-Employed. The Artist is solely responsible for my work. It is NOT the responsibility of Art Bound Studio to make any; negative reactions, faults or errors from my Artist correct in any way.

I acknowledge tattoo applications are painful and if I can’t finish the procedure, no refund will be given.

I agree to the above terms, and understand my information will not be sold or traded to any third party by Art Bound Studio.
